Gynecomastia Grades: How to Identify and Treat Each Stage
Gynecomastia can be classified into four grades, each with its own set of symptoms and treatment options.
Learn how to identify and treat each stage with this guide.
Gynecomastia grade is a medical tool used by plastic surgeons to assess the severity of Gynecomastia. The seriousness of Gynecomastia refers to the extent or degree of breast enlargement in males. Gynecomastia can range from mild to severe, and the severity can impact the complexity of the surgical procedure and consequently affect the cost.
The most commonly used grading system is based on the Simon classification, which includes the following grades:
Grade 1 Gynecomastia
Grade 2 Gynecomastia
Grade 3 Gynecomastia
Grade 4 Gynecomastia
Take Away: The grading helps the surgeon assess the severity of Gynecomastia and determine the most appropriate treatment approach, whether it involves liposuction, gland excision, skin tightening, or a combination of techniques. More severe cases may require additional procedures and a longer recovery period.

Gynecomastia grades are a medical tool used extensively by plastic surgeons to assess the severity of the disease; hence, they help plan the treatment. Individuals with Gynecomastia are more psychologically disturbed. Apart from the physical problem of the presence of breasts in the chest region, they feel depressed and lose confidence mostly because of the awkward appearance of the chest. Hence, any patient bothered about the appearance and shape of the chest needs treatment.
The gland is predominantly more developed in grade 1 and grade 2 gynecomastia. In grade 3 and grade 4 gynecomastia, apart from the development of the gland, there is also fat deposition in the chest, giving the appearance of female breasts. Hence, the treatment options are for grade 1 and grade 2 Gynecomastia; gland removal is sufficient. Apart from gland removal, even liposuction needs to be combined to improve the aesthetic appearance of the chest in both grade 3 and grade 4 gynecomastia.

There is no guarantee that it will resolve completely without intervention. Gynecomastia that develops during puberty may resolve within few years as hormone levels stabilize. However, surgical intervention is considered if grade 1 gynecomastia persists or causes significant distress as they will not disappear on its own. Gland removal is advised through a keyhole approach.
Yes, Grade 2 Gynecomastia is completely curable by surgery. Procedures such as liposuction and surgical removal of the excess breast tissue (mastectomy) are commonly performed. These surgical procedures aim to remove the enlarged glandular tissue and reshape the chest to achieve a more masculine contour.
The natural regression of Gynecomastia depends upon the cause of the condition, as in pubertal Gynecomastia. However, it’s important to note that Gynecomastia may not naturally regress or shrink in all cases. If Gynecomastia persists, surgical interventions, such as liposuction or removal of the excess breast tissue, need to be considered.
The answer is NO. Gynecomastia caused mainly by excess fat deposition (pseudo-Gynecomastia) may be responsive to exercise and weight loss. Remember that exercise alone may have a limited impact on glandular tissue enlargement, which is the primary characteristic of Gynecomastia.
